Pines to Palms offers Care Management and Mental Health Therapy services. Often times, while working through the process of healthcare, it is common to have emotions that are difficult to manage. Likewise, while uncovering emotional difficulties, often times it requires assistance handling specific tasks related to your progress. We are able to offer both services to help our clients thrive without barriers.
We have experience in private and non-profit settings for both Care Management and Mental Health Therapy, and are dedicated to helping our clients find their joy. As Certified Care Managers and Professional Members of the Aging Life Care Association, our goal is to help by overseeing needs ranging from crisis intervention, working with physicians, hospital and discharge planning, and helping to provide education and guidance for care and future planning.
With Social Work, Human Services, and Clinical Backgrounds, we focus on the person as a whole, to help guide you to move with new changes and past some of your darkest times. Life can feel overwhelming, confusing, and as if you will never be able to look past your current situation. But, there is hope. Our goal is for you to feel hope at the end of each interaction while tackling the problems you are facing today, together, while walking with you on the journey of your healing.
